Please review our policies and print and fill out applicable forms prior to your visit, to save time at your visit.

Medical History Forms

You will be given a "bubble form" to fill out just prior to your visit, which will ask you many questions about your general health. This can't be mailed ahead of time because it needs to not bend in order to be accepted by our computer scanner.

If you have specific problems, it is helpful if you can fill out the following forms ahead of time and bring them with you to your visit.

Scheduling and Arrival

Welcome new patients, and welcome back existing patients! We look forward to helping you with your healthcare needs.

We request that you arrive 30 minutes prior to your scheduled appointment time if you are a new patient or have a procedure scheduled, and 15 minutes ahead of time if you are an established patient here for an office visit. This allows time to fill out or update your paperwork, pay any applicable copays and estimated out of pocket costs, fill out your medical paperwork, have your vitals taken and your history entered into the computer and available for your provider.

This can’t be mailed ahead of time because it needs to not bend in order to be accepted by our computer scanner (although it can be completed alternatively through our Patient Portal).

In addition, please see our Medical History Forms and print and complete as many as are applicable (alternative you may complete this information securely online through our Patient Portal).

We may need to reschedule your appointment if you arrive at exactly the time you are scheduled to meet with the provider, since by the time you have completed these items, you will actually be late for the appointment and may cause the clinic to run late.

Try to be as descriptive as possible when scheduling your appointment. We realize some of these gynecologic problems are personal and may be difficult or embarrassing to describe to the patient coordinator who answers the phone, but we do run into problems running behind when issues come up as "by the way…" at the end of your annual exam. We most certainly don’t mind addressing problems along with preventative care, but do appreciate being able to predict it in the schedule.

Sometimes our clinic runs late because problems become evident at the visit that weren’t predicted when we were scheduling and take more time to sort out than was scheduled, or because patients arrive late and we try to accommodate. It may be a good idea to call ahead of your arrival time to see if we are running on time or not, especially if you have a time crunch that day.
