Within a few weeks, we hope to be able to have our online system of registering as a new patient available. This system will allow you to fill out your medical history online via our secure interactive system. Once it has been reviewed by Dr. Ravasia, it will become part of your medical record, and will be available at your subsequent appointment(s). It will also allow us to enter your demographic information ahead of time, to save time with your check in when you arrive.

In the meantime, the forms that new need to register as a new patient are listed below, and can be printed and filled out to bring with you to your appointment. Alternatively if you wish to fax them to us ahead of time at (509) 789-9013, it will help save some time for you on the day of your visit.
